OUR POLICIES

Patients are seen by appointment only.
  
New patients should arrive 15 minutes prior to appointment time to complete registration process for first-    time patients.
 
Established patients should arrive 10 minutes prior to appointment time.
 
Please bring actual medication bottles or list of medications.
 
Please call 24 hours ahead of your appointment if you need to re-schedule.
 
If you have a last minute emergency, which prevents you from keeping your appointment, please call.
  
Medication refills will be processed with in 24 business hours after receiving a request from the pharmacy. Control medications will not be refilled without an office visit.
 
Extended phone consults or after-hours and weekend calls resulting in telephone treatment may be billed as a telephone visit, this will be the patients responsibility.

It is the patient’s responsibility to notify the office when insurance and billing status has changed.
